Nate, an obnoxious sixteen-year-old chess prodigy (Nick Lane) collapses after suddenly losing control and beating up a fellow teen (Ben Bledsoe) he had just beaten at chess. He first complains about head pain, but other symptoms, hypogonadism, organ failure, and chemical imbalance which may explain Nate’s rude personality, arise. Chase finds HPRT enzyme deficiency, which could mean Kelley-Seegmiller syndrome. Cameron says Kelley-Seegmiller victims self-mutilate, chew their lips, and bang their heads against the wall. [This is incorrect; what Cameron describes is actually the Lesch-Nyhan syndrome, which is total HPRT deficiency. Kelley-Seegmiller syndrome means partial HPRT deficiency which does not produce the neurologic and psychotic symptoms of the Lesch-Nyhan syndrome.]

At the Princeton-Plainsboro Teaching Hospital clinic, House tells Wilson of the conditions for bringing Cameron back: a dinner date, which bewilders and excites Wilson, knowing Cameron is affectionate for her boss. As House leaves, he bumps into Harvey Park (John Cho) in the waiting room, whom he insults when he believes that Harvey spilled a sample of urine onto his clothes. Prodded by Wilson (who informs House that the spilled liquid was really apple juice) House reluctantly apologizes, until he discovers Harvey’s right pupil is blown, resulting from a stroke.

A diving competition is interrupted when a man with virulent bacterial meningitis collapses and causes an outbreak scare. In the beginning of the show House is seen at Cameron’s house trying to persuade her to return to the team after Edward Vogler left. Partially in an effort to avoid assisting with the quarantining, House examines a young diver named Mary (Skye McCole Bartusiak) from the meet, whose symptoms don’t fit recent-onset meningitis. Foreman is assigned to help House but Chase, probably as punishment for betraying him to Vogler, is sent to look up all potential causes for the most basic of Mary’s symptoms: neck pain (which only happens when she twists her neck side to side).
